A major heart procedure used to replace damaged aortic valves without open-heart surgery has been at the center of a debate over what drug patients should take afterward. A randomized clinical trial published in JAMA examined whether anticoagulation therapy alone performed better or worse than antiplatelet therapy alone following transcatheter aortic valve implantation, a procedure known as TAVI.
TAVI has become one of the most common cardiac interventions for patients with severe aortic stenosis, particularly older adults who cannot tolerate traditional surgery. After the procedure, doctors typically prescribe blood-thinning medications to prevent clots from forming on or around the new valve. The question of which type of drug works best has not been fully settled.
The trial, called ACASA-TAVI, was designed to produce direct evidence comparing the two drug classes head to head. Anticoagulants work by interfering with the clotting process itself, while antiplatelet drugs such as aspirin reduce the tendency of platelets to clump together. Both approaches carry risks, including bleeding, and neither has been universally accepted as the standard of care following TAVI.
The study's randomized design gives it more weight than observational research, which can be skewed by differences in how patients are selected for one treatment or another. Randomized trials assign participants to treatment groups by chance, which helps ensure the groups are comparable and that differences in outcomes are more likely due to the treatments themselves.
The findings from ACASA-TAVI are expected to inform clinical guidelines for post-TAVI drug management. Cardiologists have long sought clearer direction on this question, as the choice between drug types affects both clotting risk and bleeding risk in a population that is already medically complex.
