The findings point to a continuing challenge in preventive cardiovascular care, where a proven medication is frequently not taken as directed.
Statins are among the most commonly prescribed drugs in the world. They work by reducing low-density lipoprotein cholesterol, which is a primary driver of the artery plaque buildup that leads to heart attacks and strokes. Clinical evidence for their effectiveness in high-risk patients is extensive. Despite that, many patients do not take them consistently, or stop taking them altogether.
The JAMA report found small gains in adherence, meaning that slightly more patients are now taking their statins as prescribed compared to earlier measurement periods. But the word persistent in the report's framing carries weight. The gaps have not closed in any meaningful way, and a substantial portion of patients who would benefit from the medication are not getting that benefit.
Adherence problems with statins are well documented. Some patients stop because of side effects, real or perceived, including muscle pain. Others stop because they feel well and do not see the immediate benefit of a drug preventing something that has not happened yet. Cost and access can also play a role, depending on insurance coverage and the specific medication prescribed.
The report does not suggest a single fix. Adherence is shaped by a combination of patient behavior, provider communication, and structural factors in the health system. Small improvements over time suggest that some interventions are working, but the scale of the remaining gap means the problem is far from solved.
For patients at high cardiovascular risk, consistent statin use can meaningfully reduce the chance of a heart attack or stroke. The JAMA findings suggest that gap between what is prescribed and what is actually taken remains one of the more stubborn problems in preventive medicine.
