The head of Tennessee’s prison system is stepping down after a failed execution of death row inmate Christa Pike on Wednesday. Governor Bill Lee announced the resignation of Commissioner Frank Strada on Saturday.
Strada had been in charge of corrections in Tennessee since 2023 and was previously a corrections official in Arizona. He will leave his post later this month.
Pike was given two doses of pentobarbital during the execution but did not die. She was then transported by ambulance to a Nashville hospital where she remained unconscious and on a ventilator. Pike’s legal team said the event was cruel and torturous, describing what happened as a systemic failure.
Authorities have not yet explained why the drugs did not work as expected.
Pike’s attorney Randy Spivey witnessed the procedure from inside the execution chamber. Pike’s attorneys said in court filings that problems with the IV lines may have caused pentobarbital to enter her body tissue instead of her bloodstream.
Pike was sentenced to death in 1996 for the 1995 killing of Colleen Slemmer. The case drew national attention and sparked debate over the death penalty in Tennessee.
Strada’s resignation comes amid growing scrutiny of how executions are carried out in the state. Critics have pointed to his past work in Arizona and said he failed to address known issues with execution protocols. Pike has previously expressed remorse in a petition for clemency, saying she understood the gravity of her actions.
The failed execution has led to calls for a full review of how executions are conducted in Tennessee. Governor Bill Lee has ordered an independent review and paused all executions in Tennessee through the end of 2026. It is unclear if the state will try again to execute Pike.
Pike’s case has drawn attention not only for the execution failure but also for the broader question of how death sentences are enforced.
The incident marks a significant moment in Tennessee's history with capital punishment. Strada’s resignation is the first major personnel change since the event occurred, the South China Morning Post reported.
Pike’s failed execution was the second execution attempt halted in Tennessee this year after officials were unable to establish IV access for Tony Carruthers in May. Strada said he believes the department followed established protocol and that he will cooperate with the independent review.
